Skin Tightening in Katonah, NY

When patients come to Form & Function asking about skin tightening, one of the first things I explain is that there isn't one treatment that is right for everyone.
Loose skin can be caused by many things. We lose collagen and elastin as we age. Weight loss can leave skin without the volume that once supported it. Hormonal changes affect skin quality. We lose muscle as we get older, and changes in the fat and muscle beneath the skin can make laxity appear even more pronounced.
So rather than simply asking, “How do we tighten this skin?” I prefer to look at why it looks loose in the first place.
That is also why we have several different technologies at our Katonah office. NeoGen PSR, EXION, EXION RF Microneedling, EMFACE, Emsculpt NEO and CellSound all do something different. In many patients, using the right combination gives us a much more natural result than trying to make one device do everything.
NeoGen PSR: Regenerating the Skin With Nitrogen Plasma
NeoGen PSR is one of my favorite technologies because it approaches skin rejuvenation in a very different way.
NeoGen is not a laser and it isn't radiofrequency. It uses medical-grade nitrogen that is converted into plasma and delivered to the skin in controlled pulses.
That energy creates a controlled thermal effect in the tissue beneath the epidermis. This triggers a cellular response that stimulates fibroblasts and encourages the production and remodeling of collagen and elastin.
In other words, we're asking the skin to repair and rebuild itself.
This makes NeoGen PSR particularly useful for crepey skin, fine lines and wrinkles, laxity, sun damage, uneven texture and aging around the eyes and mouth.
I also like the fact that we can adjust the intensity. A patient who wants gradual improvement with less downtime can be treated differently from someone who wants a more aggressive regenerative treatment and is willing to have a longer recovery.
The improvement doesn't end when you leave the office. Collagen and elastin remodeling continues over the following weeks and months, so the skin can continue to improve long after the treatment itself.
EXION: Radiofrequency for Face and Body
EXION gives us another way to address skin laxity.
For patients who want a comfortable, non-invasive treatment without needles, EXION Face uses radiofrequency energy to heat tissue and stimulate collagen remodeling.
We can use it on areas such as the face, jawline and neck, particularly when the concern is mild to moderate laxity and the patient wants little or no downtime.
EXION Body allows us to use radiofrequency-based treatment on larger areas such as the abdomen, arms and thighs. This can be especially helpful after weight loss, when the skin may not retract as much as a patient hoped.
Then there is EXION RF Microneedling, which is a different treatment altogether.
With RF microneedling, very fine needles enter the skin and deliver radiofrequency energy at controlled depths. I tend to consider this when we're trying to improve not only laxity, but also problems such as acne scars, pores, wrinkles or uneven texture.
EMFACE: Sometimes the Problem Isn't Just the Skin
The face is a good example of why I don't think skin tightening should be approached with a single device.
As we age, the changes we see in the mirror aren't happening only in the skin. The muscles and other structures underneath it change too.
EMFACE allows us to address both.
It combines synchronized radiofrequency with HIFES™ muscle stimulation. The radiofrequency component works on the skin and connective tissue, while the electromagnetic stimulation targets selected facial muscles.
There are no needles and no incisions.
For the right patient, this can create a very natural-looking improvement because we aren't adding volume or trying to change the face. We're working with the patient's own facial structure.
We may also combine EMFACE with NeoGen PSR when we want to address both the underlying muscular support and the quality of the skin itself.
Emsculpt NEO: Don't Forget the Muscle
The same principle applies to the body.
If someone loses a significant amount of weight and is left with loose skin and very little underlying muscle, tightening the skin alone may not give them the result they're looking for.
Emsculpt NEO combines radiofrequency with high-intensity focused electromagnetic energy to address both muscle and fat.
The electromagnetic energy produces very powerful muscle contractions, while the radiofrequency component contributes to the treatment's effects on muscle and adipose tissue.
This can be especially useful for the abdomen, buttocks, arms and legs when improving muscle tone and body contour is part of the goal.
I think this has become increasingly important as more people achieve substantial weight loss. We don't want patients simply to become smaller. We want them to preserve and build muscle and become stronger.
CellSound: A Different Approach to Body Composition
CellSound is another technology we use, but for a different reason.
Rather than intentionally destroying fat cells through freezing or heating, CellSound uses therapeutic ultrasound to increase circulation and encourage adipose tissue to release stored fat content.
We think of it less as a traditional spot-fat treatment and more as part of a regenerative approach to body composition.
It can be used over broader areas of the body, and we objectively follow changes using calibrated imaging and measurements.
CellSound also fits well with our overall philosophy because treatment doesn't happen in isolation. We encourage exercise, sensible nutrition and good metabolic health to help patients maintain and build upon their results.
Why We Often Combine Treatments
This is where having several technologies becomes useful.
A patient who comes in after weight loss, for example, may have loose abdominal skin, reduced muscle mass and remaining areas of excess adipose tissue.
Those are three different problems.
We might use EXION to address skin laxity, Emsculpt NEO to build the underlying muscle, and CellSound to help address body composition.
A facial patient may have crepey skin and loss of facial support. NeoGen PSR may be appropriate for collagen and elastin remodeling while EMFACE addresses the muscular foundation beneath the skin.
Another patient may have laxity combined with acne scarring or textural changes, making EXION RF Microneedling a better choice.
There isn't a formula that applies to everyone, and I think that's important.
The technology should be chosen for the patient—not the patient for the technology.
Skin Tightening After Weight Loss and GLP-1 Therapy
We're seeing more patients who have successfully lost significant amounts of weight, including people using GLP-1 medications, who are now concerned about loose skin, changes in their face or loss of muscle.
This deserves more than a cosmetic answer.
When appropriate, we look at body composition, muscle mass, skin quality, nutrition, protein intake, exercise and metabolic health along with the patient's aesthetic concerns.
For some patients, the answer may be skin tightening. For others, building muscle needs to be a priority. Often it is a combination.
The goal isn't simply to look better after losing weight. It's to emerge from the process healthier, stronger and with a body composition that can be maintained.
Which Skin Tightening Treatment Is Right for You?
That depends on what we see when we examine you and what you want to accomplish.
If the primary issue is crepey or aging skin and loss of collagen and elastin, we may discuss NeoGen PSR.
For non-invasive radiofrequency skin tightening, EXION may be appropriate.
If we need to address texture, scars and laxity together, we may consider EXION RF Microneedling.
When facial aging involves the muscular foundation, EMFACE gives us another option.
If body contouring requires building or preserving muscle, we may incorporate Emsculpt NEO.
And when adipose tissue and overall body composition are part of the picture, CellSound may be useful.
Sometimes one treatment is enough. Sometimes the best result comes from combining technologies over time.
